Reserved spots for students will only be held for the first week of school. If a student is unable to begin school by the second week of the school year, the spot & fee will be forfeited and another waiting student will be contacted.

Consent to Treatment/Health Insurance Information Form
Physical Examination Record--Physical cannot be older than 6 months. Every new students needs one.
Immunization records: DTP-DTaP-DT-Td 4 or more doses, Polio (OPV/IPV) 4 doses or OPV or IPV. .MMR 2 doses, Hepatitis B 3 doses for entrance into kindergarten
